Permanant position as the Clinical Lead/Manager, You will be responsible for providing clinical leadership, advice, coordination, planning, review and assessment of all aspects of clinical delivery at the facility to ensure that the safety and wellbeing of residents and staff is constantly maintained. This role will be with the multiple Aged Care Facilities in Rockhampton.

This role entails but not limited to - 

• Provide direct resident care and nursing services with recognised nursing practice and procedure
• Working in collaboration with the Care Manager, you will be effectively managing team members and volunteers involved in care and service that support consumer daily living requirements
• Responsible for providing support and assistance to maintain high standard of services for residents

About you

• Registered Nurse with current AHPRA registration (mandatory)
• Aged Care Background 
• Experience of Management in Aged Care (Preferred)
• Experience with Aged Care electronic care systems
• Sound understanding of the Aged Care Accreditation standards and AN-ACC processes
• Experience working with older people and their families or representatives

Clinical Lead (Permanent - Full time) - Rockhampton, QLD 

As the Clinical Lead you will support the care and service team to ensure our residents are receiving excellent care. 

What you'll be doing

• Carrying out comprehensive nursing assessments for individuals and groups, and formulate care plans that provide optimal quality of life for the residents
• Coordinating and monitoring the planning, implementation and evaluation of service outcomes, liaising with the broader team on care needs
• Providing leadership and support to team members, assisting with orientation of new members, identifying learning needs and providing training and development.
• Participating in the identification and implementation of continuous improvement initiative

Care Manager (Full Time Permanent)

This Aged Care Facility is looking for an experienced Care Manager to join the team in Rockhampton, QLD!

About the role

The purpose of this role is to support the Facility Manager in the clinical management of health and clinical care outcomes of the residents in the facility. 

The duties of the role entails but not limited to:

• Provide supervision to clinical staff to ensure evidence-based practice is implemented
• Oversee the implementation of person-centred care and provide leadership to the team
• Document and report on clinical and service outcomes
• Oversee and ensure that infection control principles and procedures are maintained
• Facilitate communication between staff and resident/representative to support partnering in care
• Maintain working relationships with all care staff, medical practitioners and allied health professionals, ensuring documentation supports and optimises the Aged Care Funding Instrument (AN-ACC)

About you

To be successful in the role, you will possess:

• Registered Nurse with AHPRA registration
• Prior experience in aged care or an ability to demonstrate relevant experience
• Experience in leadership roles or opportunities/skills
• Ability to assess, plan, deliver and evaluate care for residents
